Conditions
- The $59.00 basic kit rental fee and any upgrade rental fees will be
charged to applicant's credit card at time the order is received. For
'upgrade tapes only'
orders, a shipping surcharge of $15 and any upgrade rental fees will be charged to you
credit card when your order is received. The CHILDBIRTH CLASS TO GO kit will arrive by Friday and must be
available for Pick-up no later than 12 Noon on Monday.
- Arrangements for additional rental days must be made prior to
shipment. The rental period may be extended in 2-day increments for $30.00 per two
days. If prior arrangements are not made and the kit is not available for pick-up on
Monday at 12 Noon, late charges will be incurred.
- Any kit not returned to FemmEd, Inc. within these three (3) days will
result in additional Rental Fees in the amount of $50.00 per day. At the option of FemmEd,
Inc., items not returned may be converted to sale. Applicant is responsible for full
retail price of tapes not returned, plus all rentals and extended viewing fees as well as
other related charges arising from the rental prior to the conversion of sale.
- Applicant agrees to immediately notify FemmEd, Inc. by telephone of
damages or non-delivery of CHILDBIRTH CLASS TO GO kit
on the day the kit is
scheduled to arrive.
- Loss of kit/upgrade tapes or damages by applicant will result in
additional charges to applicant's credit card to cover replacement costs of the basic kit and
any upgrades.
- Applicant agrees that all tapes are property of FemmEd, Inc.
- Additional upgrade tapes may be added to the basic kit at a rental fee of
$7/tape. Extended rental fees are $4/tape per 2-day increment. Like the basic kit fee of $59,
charges will be incurred at the time the order is received.
- All cancellations must be received in writing at least seven (7) days in
advance either by faxing to 1-877-FemmEd9 or E-Mailing to childbirthclasstogo@yahoo.com.
Applicant acknowledges and accepts full responsibility and
guarantees payment for all items rented through this application, whether or not used by
applicant.
Applicant agrees that FemmEd Inc. may pursue all avenues of
collection including that of collection agencies and authorizes FemmEd Inc. to prepare and
submit credit charge slips using the charge cards listed above to recover any and all
charges and unpaid amounts due, and reasonable legal fees resulting from CHILDBIRTH CLASS TO GO kits not returned.
